Alcon has announced its launch of the first fully personalized LASIK treatment, wavelight plus, in the US and Canada.

 The new treatment utilizes ray tracing technology through the Sightmap diagnostic device, which considers a patient’s individual eye shape and vision needs, according to a news release.

wavelight plus was first launched in China, with several European and Asia-Pacific markets following suit. The commercial launch in Canada is late 2025.

“wavelight plus marks an exciting new chapter in Alcon’s journey to provide personalized precision in laser eye surgery,” said Sergio Duplan, Alcon senior vice president and president, Americas, in the release. “For the first time, surgeons in the US and Canada will have the opportunity to deliver a fully tailored refractive treatment that takes into account the unique features of each patient’s eyes.”

By using over 100,000 data points, wavelight plus works to create a 3D model, or “Digital Eye Twin,” of each patient’s eyes with a combination of ocular wavefront, biometry, and corneal tomography measurements. The 3D model then guides a highly personalized procedure with the goal of improving visual acuity and correcting more complex vision problems, according to Alcon. Additionally, the measurements are completed without nomogram input, which streamlines planning time.

“Patients come to my practice with increasingly high expectations for their vision after surgery. With wavelight plus, I can now offer a premium, personalized treatment with a high probability of achieving visual outcomes surpassing 20/20 vision, and even pushing the boundary of visual possibilities,” said Mark Lobanoff, MD, refractive surgeon and CEO of OVO LASIK + LENS in St Louis Park, Minnesota, in the release. “This not only benefits my patients but also provides my colleagues and me with streamlined surgical planning and exceptional diagnostics through the Sightmap.”

Real-world clinical evidence supports the efficacy of the new treatment, with 200 patients undergoing wavelight plus LASIK involved in the study. Specific findings reported 100% of myopic eyes achieving uncorrected distance visual acuity (UDVA) of 20/20 at 3 months, 89% of myopic eyes achieving postoperative UDVA of 20/16, and 50% achieving 20/12.5 at 3 months. Additionally, 98% of eyes achieved postoperative UDVA within 1 line of their preoperative corrected distance visual acuity (CDVA) by 3 months, and 94% achieved the same or better CDVA than their preoperative CDVA by 3 months.

Alcon announces launch of personalized LASIK treatment in the US and Canada

The American Academy of Optometry (AAOpt) has announced its award recipients for 2025. From international to academic recognitions, AAOpt is spotlighting a diverse selection of optometrists specializing in a wide range of modalities. The organization stated that award winners will be recognized during the Academy 2025 meeting in Boston, Massachusetts, from October 8 to 11.

Results from a recent study published in 

were found to reinforce previous clinical studies that have demonstrated that Xiidra (lifitegrast ophthalmic solution; Bausch + Lomb) 5% significantly reduces symptoms of eye dryness over 12 weeks.

 The investigator-initiated, prospective, open-label study evaluated whether end-of-day comfort and dryness in symptomatic soft contact lens wearers improved after using Xiidra twice a day for 12 weeks while wearing their usual contact lenses, according to a news release.

Xiidra is a prescription eye drop used to treat the signs and symptoms of dry eye disease (DED) and works by targeting the source of inflammation.

“Although the use of OTC rewetting drops provides temporary relief for some wearers, their benefit is only moderate and contact lens discomfort typically worsens as the day progresses,” said Andrew Stewart, president of Global Pharmaceuticals and International Consumer at Bausch + Lomb, in the release. “This study showed that Xiidra may be a suitable alternative for symptomatic contact lens wearers who can’t achieve comfort or relieve dryness by switching their contact lenses or using OTC drops.”

A total of 40 participants were involved in and completed the study. The performance of Xiidra was assessed by comparing ratings on a visual analog scale (VAS; 0-100 scale; 100 worst) at 2, 6, and 12 weeks for end-of-day dryness and discomfort, in addition to Contact Lens Dry Eye Questionnaire-8 (CLDEQ-8) scores to baseline levels.

“An interesting observation from this study was that the mean age of the study population in this study (30.8 years of age) was significantly lower than the mean participant age in the phase 3 studies (58.7 and 58.8 years of age, respectively),” the study authors, led by Marc-Matthias Schulze, PhD, Dip Ing, principal investigator and senior clinical scientist at Centre for Ocular Research & Education (CORE) at the School of Optometry and Vision Science, University of Waterloo in Waterloo, ON, stated.

 “While a younger age in contact lens wearers is not surprising, the severity of contact lens-wear related symptoms in many of these relatively young study participants emphasizes the need for clinicians to continuously monitor and address patient-reported symptoms of discomfort and dryness, for example, by modifying the lens wear modality or lens care product or introducing OTC or prescription eye drops.”

The study met its primary end points, with a median VAS score improving by 61 and 59 points on the 100-point scale for end-of-day dryness and discomfort, respectively, after 12 weeks of Xiidra use. Additionally, the drops resulted in a significant reduction in contact lens wear-related end-of-day dryness and discomfort after 2 and 6 weeks compared with baseline. Dryness symptoms also improved in almost half (45%) of the study participants to the point where they would not have met the symptom-related eligibility criteria for the study after 2 weeks of treatment.

Additionally, CLDEQ-8 scores significantly reduced compared with baseline at the 2-, 6-, and 12-week follow-up visits. Specifically, a 6-point reduction was reported for symptomatic wearers of 17 different contact lens types after 2 weeks. These scores continued to decrease over the 12-week study period. After those 12 weeks, 90% of participants achieved clinically meaningful improvements of at least 3 points in the scores compared with baseline.

“The significant improvements in symptoms of end-of-day dryness and discomfort for contact lens wearers in as little as 2 weeks highlighted the effectiveness of Xiidra, even after a short period of use,” said Schulze in the release. “Additionally, results showed continued use of Xiidra throughout the study further improved symptoms, demonstrating that Xiidra remains an effective treatment over time and may allow contact lens wearers to enjoy their lenses throughout the entire day.”

Other VAS symptoms were also shown to improve, including burning/stinging, itching, foreign body sensation, photophobia, and pain. This improvement was reported as early as 2 weeks and continued throughout the study period. An increase in mean comfortable contact lens wear time also increased from 6.5 hours at baseline to 9.1 hours at 12 weeks. However, there were no changes in total wear time during the study.

Additionally, Xiidra was well tolerated, with the study indicating that contact lens wearers could safely use the drop.

New study demonstrates Xiidra's efficacy in alleviating discomfort in symptomatic contact lens wearers

Opus Genetics has dosed the first patient in LYNX-3, its pivotal phase 3 clinical trial of phentolamine ophthalmic solution 0.75% in treating significant, chronic night driving impairment in keratorefractive patients with reduced mesopic vision.

Night-vision disturbances, which can include glare, halos, and starbursts, are common in some patients after keratorefractive procedures such as LASIK, but no FDA-approved treatment options are currently available.

“Dosing the first patient in LYNX-3 is a significant milestone for our phentolamine program, which already has 1 approved indication for the reversal of drug-induced mydriasis, positive phase 3 data in potentially treating presbyopia, and now an ongoing phase 3 trial for keratorefractive patients with visual disturbances under mesopic, low-contrast conditions,” said George Magrath, MD, CEO of Opus Genetics in a press release from the company.

LYNX-3 is a multicenter, randomized, double-masked, placebo-controlled phase 3 trial that will enroll approximately 200 adults with documented decreased visual acuity in mesopic (low-light) conditions following keratorefractive surgery, including LASIK, PRK, SMILE, and RK. Participants will be randomly assigned 1:1 to receive once-daily evening dosing of phentolamine ophthalmic solution 0.75% or placebo for approximately 2 weeks.

The primary end point is the percentage of participants achieving a ≥15-letter Early Treatment Diabetic Retinopathy Study (ETDRS) (≥3-line) improvement from baseline in mesopic low-contrast visual acuity (mLCVA) in the study eye at day 15.

Phentolamine ophthalmic solution 0.75% is a nonselective alpha-1 and alpha-2 adrenergic antagonist to reduce pupil size. The company noted that phentolamine ophthalmic solution 0.75% blocks the alpha-1 receptors found on the radial iris dilator muscles, which are activated by the alpha-1 adrenergic receptors, without affecting the ciliary muscle, which reduces risks such as retinal tears or detachment associated with older parasympathomimetic agents.

LYNX-3 is the second of 2 pivotal phase 3 studies, and is being conducted under a Special Protocol Assessment (SPA) and follows the same study design and patient population as the first study, LYNX-2.

LYNX-2 was a randomized, double-masked, placebo-controlled trial in 199 patients who had previously undergone keratorefractive surgery and reported decreased visual acuity under mesopic low-contrast conditions. Patients were randomly assigned to receive either phentolamine or placebo, which was self-administered in both eyes nightly, treated, and observed over 6 weeks.

The trial met its primary end point of a gain of 3 lines, or 15 letters, or more of distance vision improvement on a low-contrast chart in low-light conditions after 15 days of dosing. In LYNX-2, 17.3% of patients treated with phentolamine ophthalmic solution 0.75% achieved a ≥15-letter Early Treatment Diabetic Retinopathy Study (ETDRS) (≥3-line) improvement in Mesopic Low Contrast Distance Visual Acuity (mLCVA) at Day 15, compared with 9.2% in the placebo group (

The company's phase 3 VEGA-3 trial also evaluated phentolamine ophthalmic solution 0.75% for the treatment of presbyopia and met its primary end point as announced with other positive topline results in June 2025. The VEGA-3 trial met its primary end point, with a statistically significant 27.2% of participants treated with the solution achieving a ≥15-letter improvement in binocular distance-corrected near visual acuity, with less than a 5-letter loss in binocular best-corrected distance visual acuity at 12 hours post dose on day 8, compared with 11.5% of patients on placebo (

Cataract surgery has evolved tremendously, with premium intraocular lenses (IOLs) offering patients greater visual freedom and improved quality of life. However, the success of these advanced technologies is not solely dependent on surgical precision or lens quality; it hinges on the conversations providers have with patients. Effective communication between optometrists, ophthalmologists, and their patients is critical to setting realistic expectations, ensuring satisfaction, and achieving optimal outcomes.

The crucial role of patient conversations

Premium IOLs, such as multifocal, extended depth-of-focus (EDOF), light-adjustable, small-aperture, and toric lenses, have demonstrated enhanced benefits beyond standard monofocal implants. However, they also come with potential trade-offs, such as night vision disturbances, neuroadaptation issues, or the need for refractive surgical enhancement for optimized vision or residual refractive error. Patients must be well informed about these nuances before making a decision.

The opportunity for comprehensive patient education begins with the optometrist, who serves as the patient’s primary eye care provider, knows the patient’s visual needs best, and is the patient’s trusted source of annual eye care information.

Optometrists play a pivotal role in initiating discussions about cataract surgery early, offering insights into the types of IOLs available, the advantages and potential limitations of premium options, and the realistic outcomes patients can expect. This early education ensures that by the time the patient is referred to an ophthalmologist for a surgical consultation, they have a clear understanding of their options.

A well-coordinated approach between optometry and ophthalmology reinforces patient comprehension and fosters a seamless transition through the surgical process. By working as a cohesive team, optometrists and ophthalmologists can ensure that patient education remains consistent across all stages of care. When both providers are aligned in their messaging, patients experience greater confidence in their decisions and better understand their visual journey. This collaboration enhances the overall surgical experience, leading to higher patient satisfaction and better visual outcomes.

The optometrist’s role: Educating and preparing the patient

Optometrists are uniquely positioned to establish a strong foundation for the cataract surgery journey. During routine eye exams or when a patient presents with vision complaints, optometrists should take the time to perform the following:

: Assess the patient’s lifestyle, visual demands, and ocular health to determine if a premium IOL would be a good fit.

: Explain that although premium IOLs can significantly reduce dependence on glasses, they may not completely eliminate the need for corrective lenses in all situations.

Address all ocular surface disease and/or modifiable factors that potentially affect surgical measurements

: Evaluate and treat all ocular surface disease or dry eye symptoms that could impact the surgical outcome or recovery process.

: Educate patients about potential adverse effects such as glare, halos, or the need for neuroadaptation with multifocal or EDOF lenses.

: Allow patients to express their fears or hesitations to ensure they feel confident in their choice before referral.

: Provide a detailed referral, including refractive data, ocular history, and the patient’s expectations and preferences, to the ophthalmologist.

Insights from optometrists on patient education

Optometrists must strike a balance between educating patients and managing their expectations. Many ODs find that using analogies—such as comparing multifocal IOLs to progressive glasses—helps patients understand the adaptation process. Common pitfalls include overpromising results or failing to emphasize that neuroadaptation varies from person to person. Clear and honest discussions help set up the ophthalmologist for success by ensuring the patient is already well prepared before the surgical consultation.

The ophthalmologist’s role: Reinforcing expectations and customizing the plan

After the referral, the ophthalmologist plays a vital role in confirming suitability, reinforcing prior discussions, and fine-tuning the surgical approach. The most successful outcomes occur when ophthalmologists perform the following:

: Ensuring consistency in messaging reassures the patient and builds trust. This is highly dependent on open communication between the primary eye doctor and the ophthalmologist.

: Topography, tomography, macular OCT, and aberrometry can help rule out underlying comorbidities and determine the best IOL choice for the patient.

: Readdress ocular surface health, manage any preexisting conditions, and personalize the lens selection based on patient lifestyle, expectations, and ocular health status.

: Remind patients that neuroadaptation takes time and that their vision may continue to improve post surgery. Also, review the possible need for the enhancement of vision if residual refractive error impedes optimized vision or lens exchange with vision outcome achieved does not reach the goal planned.

: Offer the patient the option of comanagement to maintain continuity of care with their primary eye doctor. Reinforce that comanagement with their optometrist to ensure optimal healing and the fine-tuning of vision, as needed.

Ophthalmologists’ perspective: Managing expectations and avoiding pitfalls

Despite thorough preoperative counseling and precise surgery, some patients with premium IOLs may experience postoperative dissatisfaction, often due to residual refractive error, dry eye, neuroadaptation challenges, or unrealistic expectations. In a comanaged setting, these scenarios, if not handled properly, can strain the patient-doctor relationship and test the trust between optometrist and surgeon. From the ophthalmologist’s perspective, anticipating these scenarios and proactively aligning with the referring OD on how to manage them is critical to preserving both outcomes and reputation. Here’s how to navigate these challenging postoperative moments:

 Prepare both the patient and referring OD for the possibility of visual fluctuation or adjustment issues, especially with multifocal or EDOF lenses. The added discussion prior to surgery is critical in normalizing the time it may take for some patients to become accustomed to their new vision quality and performance. It is also important to explain to patients that the spectacle independence expected is not guaranteed across all distances. Advanced lens options offer spectacle independence in most daily activities, but the patient may still require spectacle assistance or extra light in some settings.

» Establish a clear postop communication loop:

 Ensure the OD flags dissatisfaction early so the surgical team can intervene, whether it’s a refractive touch-up or reassurance. A dissatisfied patient must feel heard and validated early. It is also important that they do not feel abandoned by their surgeon.

 Discuss potential enhancement options, such as LASIK, YAG capsulotomy, or lens exchange, during the preoperative consultation, and revisit them postoperatively if visual concerns arise. Establishing and communicating a clear, stepwise approach to managing suboptimal outcomes with both the patient and optometrist demonstrates a thoughtful care plan and often helps to alleviate patient anxiety.

 When expectations are not met, avoid defensiveness; focus on collaborative problem-solving between the OD and MD to maintain patient trust. Reinforce to the patient that their satisfaction and visual outcome remain the shared priority. A unified and empathetic approach can often turn disappointment into renewed confidence in care.

 Thorough records of preoperative counseling, lens choice rationale, and patient preferences can be invaluable if dissatisfaction escalates. These notes serve as a clear reference point to remind both the care team and the patient of the informed, shared decisions made prior to surgery. They also provide a strong foundation for addressing concerns with empathy and transparency. In some cases, detailed documentation can help defuse tension by demonstrating that the patient’s goals and concerns were heard and thoughtfully considered throughout the process.

Managing expectations for long-term satisfaction

Once the patient has transitioned back to the care of their comanaging optometrist for postoperative follow-up, it is essential to continue patient education and reinforce the key aspects of the surgical journey. Comanagement between optometrists and ophthalmologists plays a critical role in ensuring long-term patient satisfaction following cataract surgery. Regular follow-up appointments, comanagement discussions, and shared patient notes help keep the optometrist and ophthalmologist aligned, ensuring that both are informed and working toward the same goal.

By working together as a team, both providers can reinforce consistent messaging, address concerns, and ensure patients fully understand the presurgery and postsurgery experience. A major challenge in premium IOL success is ensuring that patients understand their postsurgery experience. Unrealistic expectations can lead to dissatisfaction, even when clinical outcomes are excellent. By taking a collaborative approach, optometrists and ophthalmologists can align their messaging, ensuring that patients fully grasp the benefits and limitations of their chosen IOL.

This continued collaboration facilitates a smooth recovery period, when any issues related to vision clarity, neuroadaptation, or healing can be addressed promptly.
