Key clinical recommendations included performing thorough retinal examinations, particularly under dilation, to identify potential surgical risks. 

Majcher stressed the significance of multimodal imaging techniques like macular OCT, fundus autofluorescence, and wide-field color fundus photography to detect subtle retinal pathologies. Special attention was given to patients with specific conditions. For instance, those with diabetic retinopathy require extra preoperative screening, including macular OCT to check for diabetic macular edema. Patients with macular diseases might benefit from monofocal or light-adjusted intraocular lenses instead of diffractive premium IOLs. Majcher also recommended close communication between eye care professionals, considering low vision rehabilitation referrals, and using large-print educational materials to support patients with reduced vision. The presentation underscored the complexity of managing patients with retinal diseases undergoing eye surgery. By combining thorough examinations, advanced imaging, realistic patient counseling, and interdisciplinary collaboration, eye care providers can develop more personalized and effective treatment approaches. 

Majcher concluded by emphasizing that successful patient care requires a holistic, collaborative approach that considers individual patient needs, potential surgical risks, and long-term vision management strategies.

CCS 2025: Getting to know the refractive surgery patient population

In a Collaborative Care Symposium (CCS) 2025 presentation titled "Refractive Lens Exchange and Refractive Cataract Surgery," Mitch Ibach, OD, FAAO, spoke alongside Dagny Zhu, MD, discussing lens-based refractive surgical options for patients, particularly focusing on addressing vision correction needs in an aging population. In an interview with 

Ibach highlighted several key considerations in modern vision correction strategies, including patient demographics and motivations and surgical approach, among others.

Ibach highlighted that the refractive surgery patient population consists of an aging US population seeking independence from glasses and contacts; patients who are interested in refractive surgery, but often at a life stage where corneal procedures might not be optimal; and patients with a desire to reduce dependence on corrective eyewear.

The 2 surgical approaches that are centered in the presentation include visually significant cataract surgery (partially covered by insurance) and refractive lens exchange (typically paid out-of-pocket). Both approaches aim to meet patient refractive goals, whether targeting solely distance vision with reading glasses or multiple focus points, such as distance, intermediate, and close-up.

Some critical referral and patient selection considerations include:

Importance of detailed referral notes from long-term care providers

Understanding patient personality and psychological readiness

Previous experiences with mono vision in contacts can be predictive

Hyperopic patients tend to adapt more easily to advanced lens technologies

As for patient expectation management, Ibach introduced the "Rule of 95", which refers to approximately 95% independence from glasses for most activities. The realistic expectation to set for patients is that some minor corrective wear might still be necessary. A red flag for Ibach to indicate that a patient may not be the best candidate for refractive surgery is those who claim they "never want to wear glasses again."

Additionally, technological advancements for patients include specialized, customizable lens technologies, the ability to match specific technologies to individual patient needs, and tailoring surgical approach based on the patient's unique characteristics.

Ibach's key takeaway from the presentation is that modern lens-based surgical options provide sophisticated, personalized vision correction strategies that can significantly reduce dependence on corrective eyewear while maintaining realistic patient expectations.

At this year’s ASCRS meeting, the Eye Care Network sat down with Karl Stonecipher, MD, to discuss some of the most exciting clinical outcomes and technological advancements in refractive surgery—particularly in the treatment of high myopia. Stonecipher shared insights from a recent trial using the Teneo laser platform (Bausch + Lomb), highlighting its speed, precision, and patient-centered design. He delves into real-world outcomes, including a remarkable 98% to 99% satisfaction rate among patients, minimal dysphotopsias, and stable postoperative vision, even in high myopes.

Q&A with Karl Stonecipher, MD, about LASIK results in patients with high myopia

Q: What were the key figures that we were kind of excited about with regard to outcomes in this particular arm of the trial? 

So we're really looking at high myopia. So we're talking about -8. The average for the study was about -5, -5.6, but I want to go back and say what I'm excited about the laser first, and what the laser does to get the outcomes. It's a GUI interface, which is really easy to understand. My staff loves that. It's very comfortable for the patient, and it's extremely fast at 1.2 seconds per diopter. I think a lot of that has to do with the outcomes of over 98% of the patients doing extremely well and very satisfied with the outcomes, with amazing stability.

Q: What kind of dysphotopsias are we seeing post LASIK with this laser platform, and how do the patients report that and respond to it? 

Since we all know that when we first started with higher myopes, we're concerned about spherical aberration, and we're actually showing that we're getting improvement from baseline to present day. And so what happens is, patients come in, we don't see as much glare and halos. The PROWL questionnaire is obviously what we use, and we show improvements across all platforms at this high level, and we don't see dysphotopsias like we saw with a lot of those earlier platforms.

Q: Are patients satisfied with Teneo results?

Well, I can tell you that with patient satisfaction, we had some of the highest rates we've ever had: 98%, 99% saying that they would recommend it to families and friends. They would do it again if they had to. So obviously, at those levels, you can't please everybody. You can't make everybody happy. You know, you'd love to make all of them smile and walk out the door, but [I've] got to tell one anecdote. And I'm not sure if this is great, but one of the patients postop said to me, I love you almost as much as I love Jesus now. So obviously that's a good thing in terms of the outcome. So I think that we're happy that patients are happy. But we also now have these questionnaires. So they're happy in low light, they're happy in bright sunny days. They're happy in... they're getting up to go hunting or they're getting up to take their kids to school. So I think that's important.

Q: What do you like about the Teneo platform?

So the Teneo is a new platform. I like to tell you the things that I like about it. The No. 1 thing that I like about it any surgeon, especially my residents and fellows that work with me, are what...they're concerned about what do I put in the box? What do I put in the laser? So what's the manifest refraction? So we're talking about high myopia. We just put the manifest in the box. This is not nomogrammed information, and you had 20/20, 20/25 level of vision that was stable in the high 90 range. And so patients are coming back happy, and it's the surgeon's comfort. Now, don't get me wrong, I'm sure we're going to nomogram things down the road to even fine-tune that more, but this is off the manifest refraction. So we're not cycloplegia-ing these patients anymore. We're not having to do... with the nonmydriatic cameras we can look at the retina. So if I see something abnormal in the retina, then I'm going to send this patient category off to see the retinologist, because it's important at this level to make sure they don't have any holes or tears. But it's a very convenient laser, because you can come in, I can manifest you, you don't have to go through the dilation process, and get great outcomes.

. For Shamie, these procedures represent more than clinical advancements; they embody a modern, patient-centered approach to vision correction.

Refractive cataract surgery, she explained, is about leveraging today's technological innovations to reduce a patient’s dependence on glasses at the time of cataract surgery. “It’s a more contemporary, elevated approach,” she said, emphasizing the importance of engaging patients in conversations about their lifestyle and visual goals. With options like multifocal, toric, extended depth-of-focus, light-adjustable, and small-aperture IOLs, practitioners now have an expansive tool kit to match the right lens to the right patient.

Preoperative education is an opportunity for optometrists

Shamie encouraged optometrists to initiate these discussions early, educating patients on the variety of options available. While these conversations may lengthen chair time, she believes they significantly enhance the patient experience and ensure surgical success. “I spend the time with patients to really engage in what they like to do, what their visual needs are, and see if I can match-make them to the right lens,” she said.

The conversation extended to refractive surgery more broadly. While LASIK and photorefractive keratectomy remain excellent options, Shamie warned against a one-size-fits-all approach. “When you only offer those two, you tend to stretch the limits of safety,” she noted. Today’s refractive surgeon must also consider alternatives like the EVO ICL for non-LASIK candidates or refractive lens exchange for hyperopic or presbyopic patients.

Shamie highlighted the role of optometrists in identifying candidates for these alternatives, especially when patients express dissatisfaction with glasses or contact lenses. “It’s a wonderful way to really extend their trust in you,” she added.

Collaborative care is the foundation of optimal outcomes

Shamie closed with a reminder of the meeting’s founding purpose: promoting true collaboration between optometrists and ophthalmologists. Citing the vision of Robert Maloney, MD, MA, who began the conference nearly two decades ago, she reaffirmed her belief that optimal patient outcomes stem from seamless, respectful comanagement.

“The whole journey of vision recovery is optimized when there’s a clear and streamlined handoff,” she said. From preoperative education to ocular surface optimization, optometrists lay the foundation for surgical success.

Managing expectations with a personalized approach

While these IOLs significantly enhance range of vision, it’s critical to remember—and to communicate—that no artificial lens fully replicates the perfection of the natural crystalline lens. Preoperative counseling remains vital. Educating patients on what to realistically expect following surgery helps ensure satisfaction and trust in the surgical outcome.

The best surgical outcomes begin long before the operating room. Optimizing the ocular surface preoperatively enhances biometry, improves IOL power calculation accuracy, and, ultimately, contributes to better refractive outcomes. This foundational step cannot be overstated, especially in patients seeking premium IOL options.

Today’s IOLs can support a wide range of visual demands. Whether patients are tying fishing flies, changing watch batteries, reading scripture, using digital devices, or watching a ballgame with closed captions, these lenses allow for impressive functional vision. This breadth of utility boosts confidence in recommending such lenses.

A recent study involving 40 patients with bilateral implantation of next-gen IOLs revealed surprising results—average distance visual acuity exceeded 20/20, with many patients achieving nearly 20/15. Coupled with solid near vision, this performance underscores the power of modern IOL design.

As primary eye care providers and preoperative counselors, optometrists play a crucial role in identifying visual goals and lifestyle needs. Listening carefully during preoperative evaluations allows for the customization of IOL selection, aligning lens technology with patient expectations.

Eva Kim, MD, shares visual outcomes of EVO implantable collamer lens in patients with low to moderate myopia

EVO ICL in Low to Moderate Myopia: A Refractive Option Worth Equal Consideration

As the landscape of refractive surgery continues to evolve, many optometrists are seeing increased interest in the EVO implantable collamer lens (ICL) (Staar Surgical) as a primary option—not just for high 

, but now for low to moderate myopia as well. Eva Kim, MD, cataract and refractive surgeon and uveitis specialist based in Deconfnver, Colorado, spoke on EVO ICL at ASCRS 2025, in Los Angeles, California. 

One of the most significant advantages of the EVO ICL, Kim emphasizes, is its reversibility. "It’s an additive procedure," she explains. "We're placing a custom lens inside the eye to provide outstanding vision, and if for any reason a patient wants it removed, we can return the eye to its original state." This concept is particularly appealing to younger patients who may be hesitant to commit to a permanent structural change to their cornea.

Importantly, at Kim’s practice, EVO ICL is not framed as a backup or secondary option to LASIK. Instead, both procedures are presented as equally strong choices, allowing patients to weigh their benefits. Interestingly, more than 50% of patients who qualify for both procedures are opting for EVO ICL—largely due to its reversibility and non-invasive perception.

The design of the EVO ICL also contributes to a superior safety profile. One major improvement over earlier ICL models is the elimination of the need for a preoperative peripheral iridotomy, which traditionally required a laser treatment that could be uncomfortable and invasive. Without this step, the patient experience is significantly streamlined and less intimidating.

In terms of postoperative recovery, Kim notes that while visual clarity on the day of surgery may be compromised due to dilation, most patients experience a dramatic improvement in vision the very next morning. Recovery is similar in timeline to LASIK—typically 1 day to 1 week—but patients tend to report greater comfort and fewer dry eye symptoms, making the healing process smoother overall.

As optometrists guide patients through their refractive surgery options, the EVO ICL offers a highly viable and increasingly popular alternative to LASIK—even for patients with mild myopia. With its strong safety profile, reversibility, and high patient satisfaction, it deserves a central place in today’s refractive care conversation.

AOA 2024: Supercharged comanagement

In order to streamline referral efficiency for patients who may ask for or be good candidates for refractive surgery, Tracy Lynn Schroeder Swartz, OD, MS, FAAO, gives tips for optometrists in her talk "Supercharged Comanagement." She gave her talk at this year's AOA's Optometry's Meeting in Nashville, Tennessee.

 This transcript has been lightly edited for clarity.

Tracy Lynn Schroeder Swartz, OD, MS, FAAO:

Hi, I'm Tracy Lynn Schroeder Swartz. I work in a referral center in Huntsville, Alabama, and at the Congress this year I am lecturing on "Supercharged Comanagement." We are suffering a shortage of ophthalmologists, which is resulting in considerable wait times for our patients who we refer for surgery. In my hometown, patients often have to wait 3 to 4 months between the time that I referred them to the time that they actually have cataract surgery. Optometrists need to be aware of what the patient's status is prior to that referral for surgery. Now, whether that referral is for visual elective surgery like LASIK or SMILE, or cross-linking, or Phaco, or maybe phaco with MIGS, it's important to look at the patient's overall ocular health and determine if the patient is actually ready for surgery.

So I suggest using a really easy algorithm. The first question is, "Does this patient have ocular surface disease?" The second question is, "Does this patient have a normal cornea?" And then the third question is "Does this patient have cataracts?" Your patients aren't really going to be able to have any surgery if the ocular surface disease is significant or uncontrolled. We need to fix that prior to even talking about surgery with them because, for example, if you have a patient with severe keratitis and they want LASIK because they can't wear their contact lenses, it's an understandable desire for the patient. But from our standpoint, they can't really do LASIK if we can't control their ocular surface disease. So it's important to look at their keratitis, their lids, make sure that everything is as under control as you can get it before we proceed with that referral.

The second thing to consider is, does this patient have a normal cornea? Sometimes the patients will suffer from [epithelial basement membrane dystrophy, or] EBMD or maybe a pterygium that can directly impact the refractive error. And again, we can't address the refractive error; it's the cornea is the reason the refractive error is a problem. So we need to determine if the cornea needs to be corrected. For example, maybe a [phototherapeutic keratectomy, or] PTK or visually significant basement membrane dystrophy, or maybe a pterygium removal, or cross-linking for a progressive keratoconus prior to really discussing the surgery that the patient came to you about.

The last question is if the patient has cataracts. If the patient has cataracts, or maybe early cataracts, which we now call dysfunctional lens syndrome, that is also going to determine what type of surgery the patient would benefit the most from. If the patient has cataracts or don't really want to do LASIK or SMILE and if the patient has got glaucoma with cataracts, they may want to discuss MIGS with Phaco. So if you answer those 3 questions: Do they have ocular surface disease? Is a corneal normal? Do they have cataracts? Then you're going to be ready to refer that patient. It's important to remember that if you send a patient to a surgeon, and the surgeon says, "They're not ready, they need their ocular surface disease treated," you've actually just canceled someone else's cataract surgery because now your patient is going to have to go back and will have had 2 consultations for surgery instead of just one. So someone literally didn't have cataract surgery because you didn't treat their ocular surface disease and we don't want to make that mistake.

AOA 2024: Exploring options in premium IOLs and refractive surgery

At AOA 2024, Nick Bruns, OD, FAAO, discusses the evolving landscape of premium intraocular lenses (IOLs) and refractive surgery in his upcoming talk. He emphasizes the importance of listening to patients to understand their visual needs and lifestyle, ensuring the best lens match and managing expectations. Bruns highlights the success of EVO ICL, a game-changing option with minimal complications and excellent visual outcomes, particularly for patients not suited for LASIK or other procedures. He advocates for considering a range of refractive surgery options beyond LASIK to provide optimal patient care.

Yeah, I'm Nick Bruns, I practice at Summit Eye Care in Milwaukee, Wisconsin. Yeah, so tomorrow I'm gonna be giving a talk on premium IOLs, kind of the changing landscape in premium cataract and refractive surgery. And so we're gonna cover a lot of different things about kind of how there's more of a trend towards refractive outcomes. I mean, back decades ago, it was a standard monofocal lenses, and patients were understanding that they'd wear glasses. But now, today's patient in 2024 no longer is accepting, you know, wearing glasses or needing glasses full time. So we're going to talk about different lens options, the pros and cons of every one, how we try to maximize these outcomes to really meet our patient's changing lifestyle. Everybody's got different visual demands. It's kind of a weird thing to think about: How do you use your eyes? And so everybody's very different. And so every lens, trying to fit that lens to the right patient is a really important thing that optometrists really have a huge key part.

So I think the biggest tip I have for trying to make that proper match is really listening to your patient. You really have to get down to what do they want to do? What are their hobbies? What are their lifestyles? Are they a big reader? Are they very active? Are they skiers or golfers? Each lens has different, you know, different priorities and different things that it can maximize. And so trying to maximize what the lens can do to match what that patient needs is really, really important in trying to get to know your patient. So I think there's really an interpersonal relationship between doctor and patient that has to be kind of taken to the next level to match that appropriately. Number one takeaway, I think, is really listen to what your patient is telling you.

Yeah, so if patients come back, and we'd like to think that every one of our patients is just super happy and super thrilled with their their outcome and their vision, but inevitably, unfortunately, we're forced to deal with some challenging situations because not every match is perfect. We're not 100%. And so when you get that patient back, and they're just not thrilled with their outcome, we want to do a couple things. Number one, we want to advocate for our patients, we want to make sure that we're listening to what it is they're complaining about. Is it a problem with the optics of the lens? Is it a problem with maybe visual or optical discomfort? Is there any pain or dry eye sort of symptoms? Or is it more of a buyer's remorse situation? Because remember, we're taking these patients now and turning them into consumers. There is an out of pocket expense for these procedures. So they have really high expectations.

And so again, it goes back to listening. That is the number one takeaway of the entire lecture; it's going to be listen to your patient, try to hear exactly what it is. And don't run away from it. You don't want to be defensive. You know, if there's a patient complaint, it's not personal. You've got to be a teammate with your patient and try to work together to find a solution, if there is one.

And sometimes time is the best medicine. Neuroadaptation really takes weeks to months; it can take years. And so sometimes time is the best medicine. You don't have to rush to the OR or rush to the YAG laser to do a YAG capsulotomy.

So EVO ICL has been a huge game changer for us. So we've worked in Milwaukee with the Visian ICL for a long time, had fantastic outcomes. But back in 2022, when EVO was introduced, it really has opened up this whole world of refractive candidates. So the complication rates have plummeted. There's rarely any complications with EVO ICL. The visual outcomes are just impeccable. I mean, we're actually gaining lines of visual acuity from contact lenses or glasses because of where we're correcting. We're correcting vision at the nodal point of the eye, which is right at the anterior plane of the crystalline lens.

So the optics are fantastic. The complications are slim to none. And so we're really able to capture this whole portion of patients who are searching for options when maybe LASIK is not a good option, or maybe PRK is not a good option or SMILE because there are reasons that those procedures aren't 100% available for certain patients. And so we have EVO that really can open up this other subset of patients. So EVO, we kind of think of it as our more high myopic patients subset, but in reality, we have really good outcomes with mid to moderate myopes that really compete with the outcomes of LASIK. And the cool thing about EVO that patients love is that it's completely reversible. So if patients aren't really happy, never had anybody come back and ask us to take the lens out, but if they want it out, we could do it.

I think there's been a slower adaptation of it in the surgical space because of the complications that that existed with the Visian. There were complications; we saw them. There were things that did happen and they were less than, you know, fun things to go through from a clinician standpoint and a vision standpoint, obviously. And so I think there's still this sort of hesitation. Surgeons are aware of those complications and they're just a little bit gunshy. EVO has completely eliminated those complications. We don't see secondary glaucoma anymore. We don't see premature cataract formation anymore. It just the inclusion of the central [KS-]AquaPORT has eliminated all of it. A simple inclusion of a 360 micron hole has completely taken those complication risks away. So I think there's still a little bit of gunshy but if you actually look at the trends that is becoming much more and more, I think the gunshyness is going away. And so you're gonna see EVO, a lot more prevalent in the next couple of months to years. It's not going away.

My only recommendation, one of my recommendations would be to not think of refractive surgery as LASIK. Sometimes I think especially newer doctors, think of refractive surgery and LASIK as being synonyms. They are not. LASIK is a subset of refractive surgery, but there's a whole bunch of different options. EVO ICL is one of the, in my opinion, one of the best, the safest, and visually advantageous options that we have.

With an expanded program designed to foster learning opportunities and connections with colleagues, the 18th Annual Controversies in Modern Eye Care (CIME) meeting will take place at the Hilton Los Angeles/Universal City in California on May 4, 2024.
